49438 - PHYSICIAN - College Equality & Diversity Details form UAT (NEP-6780)
Within the PHYSICIAN site, add a new question to the College Equality & Diversity Details form.

49564 - EPORTFOLIO - Blank forms non-functionality after deployment including VSTS 49419 (NEP-7709)
Within ALL sites, resolve the issue that caused Blank Forms functionality to fail.
